The standard concordance for the LXX is by Hatch and Redpath.
The occurrences for agape (just the noun form) are:
2 Kings 1:26
2 Kings 13:15
Eccl 9:1
Eccl 9:6
Song of Solomon 2:4; 2:5; 2:7; 3:5; 3:10; 5:8; 7:6; 8:4; 8:6; 8:7 (twice)
Wisdom of Solomon 3:9; 6:18
Ecclesiasticus 48:11
Jeremiah 2:2
all translate the Hebrew word ahavah
