You are, of course, quite right; I was looking at a hard copy and trying to
type while reading the page numbers. I do make mistakes that way.

Yes, I see this now--it's not in the index. I still have my doubts,
however, that there was originally a -S- or that the root of QEOS was ever
QESO-. Of course, I could be wrong about that too, but I haven't seen any
evidence for it.
