Bob, my guess would be that he's describing parablepsis related to a
similarity between the -ON and the QY nomen sacrum.  The uncial omicron and
theta could be easily mistaken at a glance, as the miniscule nu and upsilon
could easily be so mistaken, but I don't think both letters in either uncial
or miniscule would be.

That's just a wild guess, though.
