Now that I have a little more time I was able to find what I'm pretty
sure you were looking for in the Harvard library catalog - a good
place to search for such things because of the size of the collection:
Codex Bezae cantabrigiensis qvattvor Evangelia et Actvs apostolorvm
complectens graece et latine svmptibvs Academiae phototypice
repraesentatvs. Cantabrigiae [i.e., Cambridge], 1899. 2 v. 34 x 28 cm.
Phototype reproduction by Paul Dujardin. The leaves of the facsimile
are numbered like the corresponding leaves of the coedex: 3 p. l. (not
numbered), 3-16, 25-104, 113-175, 177-348, 415-446, 455-502, 504-508,
510, the numberes omitted being those of leaves lost from the codex;
in the facsimile a blank leaf is inserted in each case with a note of
the number of leaves lost.
The bibliographical record includes, "Copy no.149," implying it was a
limited edition and therefore presumably even rarer than a trade
edition would have been. WorldCat lists 27 copies:

As Mr. Rodríguez noted, the diplomatic edition I wrote about earlier
was reprinted by Pickwick Press in what I think of as recent years,
1978. There's an edition published by the Vatican in 1996, but I think
its self-description as _copia esatta_ indicates it's a transcription
rather than a facsimile.

If you're curious about other editions and studies, a search by title
of "Codex Bezae" in WorldCat or a large library catalog will net you a
good number of works of possible interest.
