Problem: Montgomery County, MD had a problem with health care for pregnant, low-income women. Doctors, afraid of lawsuits, were unwilling to deliver their babies; at one point, only one hospital in the county was willing to handle these mothers. Proposed Solution: The county decided to make obstetricians working with poor women into temporary county employees, protected by the county's self-insurance program against liability lawsuits. The providers of liability insurance to the doctors agreed not to raise the rates due to these patients, since their insurance would not be required to cover any resulting lawsuits. The county also developed a plan to insure that no one hospital was overwhelmed with mothers who were unable to pay. Evaluation: The program has been successful; since its establishment, there has been a sharp increase in the number of doctors and hospitals willing to serve low-income mothers. There has been no problem with lawsuits. The county is planning to expand the program to help with the delivery of other medical services to the poor.
