NATIONAL INSTITUTES OF HEALTH NATIONAL RESEARCH SERVICE AWARDS INFORMATION STATEMENT FOR SENIOR FELLOWS Under authority of Section 487 of the Public Health Service Act as amended (42 USC 288), the National Institutes of Health (NIH) provides National Research Service Awards (NRSAs) for research training in specified areas of biomedical and behavioral research. The senior fellowship is designed to provide training for experienced scientists to make major changes in the direction of their research careers, or to broaden their scientific background by acquiring new research capabilities. These awards will enable individuals beyond the new investigator stage to take time from regular professional responsibilities for the purpose of receiving training to increase their scientific capabilities to engage in health-related research. This program is not designed for investigators seeking to prove their research potential. Awards are made to individual applicants, for specified training proposals, selected as a result of a national competition. The NRSA legislation requires recipients of support to pay back the Federal Government by engaging in health-related biomedical or health-related behavioral research and/or teaching. See page 3 for details. Title 42 of the Code of Federal Regulations, Part 66, is applicable to these awards. The Catalog of Federal Domestic Assistance numbers 13.121, 13.122, 13.306, 13.398, 13.821, 13.837 13.839, 13.846-13.849, 13.853-13.856, 13.859, 13.862-13.871, 13.880, and 13.894 are applicable to these awards. ELIGIBILITY REQUIREMENTS. Individuals must be, at time of application, citizens or noncitizen nationals of the United States, or have been lawfully admitted to the United States for permanent residence. An individual lawfully admitted for permanent residence must submit, with the application, a notarized statement indicating possession of the Alien Registration Receipt Card (I-151 or I-551). Individuals on temporary or student visas are not eligible. Individuals must have received, as of the beginning date of the NRSA appointment, a Ph.D., M.D., D.O., D.D.S., D.V.M., O.D., D.P.M., Sc.D., D.Eng., D.N.S., or equivalent degree from an accredited domestic or foreign institution and must have had at least seven subsequent years of relevant research or professional experience. The research areas in which applications are accepted are listed under attachment in this document. Individuals are encouraged to contact the appropriate institute representative listed in the attachment before submitting an application. The proposed study must be full-time and must include the conduct of research with supervision or other opportunity for training appropriate to the applicant's background and objective. Awards are not made for study leading to any of the profesional degrees (M.D., D.O., D.D.S., or similar degrees) or for residency or other nonresearch clinical training. Before submitting an application, an individual must arrange for acceptance by an institution and sponsor. The institutional setting may be a nonprofit private or public institution, including a federal laboratory. Individuals requesting foreign training must provide, in the application, justification, based on the nature of the facilities and/or training opportunity, of the particular suitability of the foreign site rather than a domestic one to the proposed research. Only in cases where there are clear scientific advantages will foreign training be supported. REVIEW. Applications will be evaluated by NIH initial review groups three times a year. This evaluation will be based on the applicant's research competence, the research training proposal, the sponsor and training environment, the reference reports, and the anticipated contribution to be made upon completion of the training to research in areas of national need. Final selection will be made by the appropriate NIH bureau, institute or division based on the initial review group's recommendation, program interests, and the availability of funds. Senior fellowship support may be requested for a period of up to 2 years. However, no individual may receive more than 3 years of aggregate NRSA support beyond the predoctoral level, including any combination of support from institutional and individual awards, without a waiver from NIH. Where necessary, the application should be accompanied by a letter requesting a waiver of the 3-year limit. STIPENDS AND OTHER TRAINING COSTS. Stipends for senior fellows are determined individually at the time of award. The amount of the stipend to be paid shall be based on the salary or remuneration which the individuals would have been paid on the date of award from their home institutions, but in no case shall the NIH stipend award exceed $30,000 per year, prorated on a monthly basis for less than 12-month awards. The level of the NRSA stipend award will take into account concurrent sabbatical salary support provided by the home institution and any other supplementation. Applicants provide preliminary budget information to NIH by completing the salary flier at the end of this information statement. The stipend is not provided as a condition of employment with either the Federal Government or the institution. Fringe benefits are not provided with this award. No allowance will be provided for dependents or for an individual's travel to a domestic training site. Individuals affiliating with foreign sponsoring institutions will be provided a single economy or coach round-trip travel fare to the training site. U.S. flag air carriers must be used to the maximum extent possible when commercial air transportation is the means of travel between the United States and a foreign country or between foreign countries. Upon request, NIH will provide funds of up to $3,000 per 12-month period to nonfederal sponsoring institutions to help defray such awardee expenses as tuition and fees, health insurance, research supplies, equipment, travel to scientific meetings and related items. An allowance of up to $2,000 is available for individuals sponsored by federal laboratories for scientific meeting travel expenses, health insurance, and tuition and fees. For award periods of less than 12 months, the allowance will be prorated on a monthly basis. PAYBACK REQUIREMENTS. Before individuals can receive an NRSA, they must sign an ageement that they will fulfill the legislatively mandated payback requirements. Recipients agree to engage in health-related biomedical or health-related behavioral research and/or teaching for a period equal to the period of NRSA support in excess of 12 months. An individual's initial 12 months of postbaccalaureate NRSA support is excluded from payback obligation. All subsequent NRSA support is fully obligated. Recipients must begin to undertake the obligated service on a continuous basis within 2 years after termination of NRSA support. For individuals who fail to fulfill their obligation through service, the United States is entitled to recover the amount of NRSA funds paid to the individual for the period for which the obligation exists, plus interest at a rate determined by the Secretary of the Treasury. REFERENCES. NIH is working to reduce the time required for completion of the receipt, referral, review, and award of individual postdoctoral fellowship(F32) and senior fellowship (F33) applications. The goal is to cut the current time of 3 to 9 months in half. Accomplishing this goal would benefit candidates and their sponsors by giving them more time for planning future reseaech training activities. To help expedite the review process, NIH is now requiring that at least three completed, sealed letters of reference be submitted with each individual and senior fellowship application. Four copies of the reference forms are included in each fellowship application kit. Candidates should: 1. Send these forms to their referees well in advance of the application submission date, and advise the referees to complete the form and return it to the candidate in a sealed envelope as soon as possible; 2. Request reference reports only from individuals who will be able to return them in time for the application submission. Consider any factor (e.g., illness or overseas sabbatical, etc.) that might cause an inordinate delay; 3. Choose individuals, other than the sponsor of the application, who can make the most meaningful comments about the candidate's qualifications for a research career; 4. If applicable, include a reference from the current mentor or immediate supervisor. If not submitting a reference from the thesis advisor orl chief of service, explain why in Item 23 of the application; 5. Where possible, select at least one respondent who is not in the candidate's current department; and 6. Select graduate or medical school referees rather than those from undergraduate schools. To protect the utility and confidentiality of reference letters, candidates are asked not to open the envelopes. The sealed enveloped should be attached to the original application. Applications with fewer than three references will be returned. Candidates reapplying (competing continuations or revised applicants) must submit new reference forms to facilitate the expedited review process. These procedures are effective beginning with the May 10, 1988 receipt date. Emphasis will be to stabilize fundamental training in the basic disciplinary are at the predoctoral level and to use specialized training at the postdoctoral lev to meet national research priorities. Training grants are a desirable mechanism the postdoctoral training of physicians and other health professionals whose doctoral training usually involves only limited research experience. For such individuals, the training may be a part of a research degree program. APPLICANT ELIGIBILITY REQUIREMENTS Domestic nonprofit private or public institutions may apply for grants to suppor research training programs. The applicant institution must have, or be able to develop, the staff and facilities required for the proposed program. The traini program director at the institution will be responsible for the selection and appointment of trainees to receive NRSAs and for the overall direction of the program. The training program must provide opportunities for individuals to carry out supervised biomedical or behavioral research with the primary objective of exten their skills and knowledge. Clinical departments or programs should have a significant relationship with basic scientists that will assure trainees with clinical backgrounds the opportunity to acquire the necessary foundation for fut investigative work. GENERAL PROVISIONS NRSAs may not be used to support studies leading to the M.D., D.O., D.D.S., D.V. or other similar professional degrees, or to support residencies, which means postgraduate training for doctors of medicine, osteopathy, dentistry, optometry, podiatry, nurses, and other individuals providing health care directly to patien where the majority of their time is spent in non-research clinical training. However, if a specified period of full-time research training is creditable towa specialty board certification, the NRSA may support such research training if th trainee has shown a clear interest in a research career. Physicians accepted fo NRSA appointment who have had little or no prior research training should be committed to at least two years of initial research training. Trainees are required to pursue their research training on a full-time basis devoting at least 40 hours per week as specified by the sponsoring institution i accordance with its own policies. Research trainees in clinical areas are expec to devote their time to the proposed research training and to confine clinical duties to those which are a part of the research training. 1 TRAINEE ELIGIBILITY REQUIREMENTS The individual to be trained must be a citizen or a non-citizen national of the United States or have been lawfully admitted for permanent residence (i.e., in possession of the Alien Registration Receipt Card I-551 or I-151) at the time of appointment. Individuals on temporary or student visas are not eligible. Predoctoral trainees must have received a baccalaureate degree as of the beginni date of their NRSA appointment, and must be training at the postbaccalaureate le in a program leading to the award of a doctor of philosophy of science or equiva degree. Individuals who wish to interrupt their medical, veterinary, dental, or other professional school studies for a year or more to engage in full-time rese training before completing their professional degrees are eligible; however, pri approval by the NIH is required before their NRSA appointment. Postdoctoral individuals must have received, as of the beginning date of the NRS appointment, a Ph.D., M.D., D.O., D.D.S., D.V.M., O.D., D.P.M., Sc.D., Eng.D., Dr.P.H., D.N.S., or equivalent degree from an accredited domestic or foreign institution. Within the frame of the program's longstanding commitment to excellence and projected needs for investigators in particular areas of research, it is important that attention al be given to recruiting individuals from minority groups that now are underrepresented nationally in the biomedical and behavioral sciences. Also, renewal applications should include cumulative information on the subsequent career development of all trainees, including information about their minority status. Also program directors should be aware of a recruitment pool in the nurse commun which may have been overlooked. NRSA program directors should make information about their programs available to the nursing profession. PAYBACK PROVISIONS Before trainees can be appointed to a training grant, they must sign an agreemen that they will fulfill the NRSA payback requirements. Recipients agree to engag biomedical or health-related behavioral research and/or teaching for a period eq to the period of NRSA support in excess of 12 months. Once an individual has ha months of postbaccalaureate NRSA support, all subsequent NRSA support is subject payback. Recipients must begin to undertake the obligated service on a continuous basis within 2 years after termination of NRSA support. For individuals who fail to fulfill their obligation through service, the United States is entitled to recov the total amount of NRSA funds paid to the individual for the obligated period p interest at a rate determined by the Secretary of the Treasury. Financial payba must be completed within three years beginning on the date the United States bec entitled to recover such amount. Under certain conditions, the Secretary of Hea and Human Services may extend the period for starting service or for repayment, permit breaks in service, or otherwise waive or suspend the payback obligation o individual. For postdoctorals, the stipend for the first year of support is determined by th number of years of relevant postdoctoral experience at time of appointment. Relevant experience may include research experience (including industrial), teaching, internship, residency, or other time spent in full-time studies in a health-related field beyond that of the qualifying doctoral degree. The stipend each additional year of NRSA support is the next level on the stipend scale. Under no circumstances may the conditions of stipe supplementation detract from or prolong the training. The Tax Reform Act of 1986, Public Law 99-514, impacts on the tax liability of a individuals supported under the NRSA program. Degree candidates who, prior to t enactment of Public Law 99-514, were able to exclude all monies received under a NRSA award from their reported income, may now exclude only course tuition, fees books, supplies and equipment required for attendance. Non-degree candidates, w formerly were able to exclude from stipends $300 a month for a period not to exc 3 years will now be required to report all stipends and any monies paid on their behalf for course tuition and fees required for attendance. These new statutory requirements are in force as of January 1, 1987. NIH is not in a position to advise students or institutions about their tax liability. In any event, changes in the taxability of stipends in no way alters relationship between NRSA fellows, trainees and institutions. NRSA stipends are not now, and never have been, salaries. Trainees supported under the NRSA are n in an employer-employee relationship with NIH or the institution in which they a pursuing research training. Tuition and fees, including medical insurance, are allowable trainee costs if su charges are required of all persons in a similar training status at the institut without regard to their source of support. Tuition at the postdoctoral level, i justifiable, is limited to that required for specific courses in support of the approved training program. Annual increments in tuition costs beyond the first of a multi-year award (generally 5 years) may not exceed six percent. Trainee travel, including attendance at scientific meetings, which the instituti determines to be necessary to the individual's training, is an allowable trainee cost. Institutional costs of up to $1,500 per year per predoctoral trainee and up to $2,500 per year per postdoctoral trainee may be requested to defray the costs of other training related expenses, such as staff salaries, consultant costs, equipment, research supplies, and staff travel. Also, an indirect cost allowanc based on 8 percent of total allowable direct costs, or actual indirect costs, whichever is less, may be requested. PERIOD OF SUPPORT Institutional grants may be made for competitive segments of up to 5 years and a renewable. No individual trainee may receive more than 5 years of aggregate NRS support at the predoctoral level and 3 years of aggregate NRSA support at the postdoctoral level, including any combination of support from institutional trai grants and individual fellowship awards. Any exception to the trainee support 3 policy requires a waiver from NIH. REVIEW PROCESS Applications are evaluated for merit by NIH initial review groups based on the following criteria: the proposed research training objectives and program desig the qualifications of participating faculty, the previous training record of the research program and its ability to attract high caliber trainees, the availabil of research support, the extent of the institutional commitment, and the availab facilities. The initial review groups, following their assessment of the qualit training grant applications and assignment of priority scores indicative of meri will comment on each applicant's plans for attracting individuals from underrepresented minority groups into the research training program. MECHANISMS OF AWARD This award may be supported through two mechanisms: the individual award and the program award. A. Individual Awards 1. The Environment Applications will be accepted from a domestic university, medical school, or comparable institution with strong, well-established research and training programs, adequate numbers of highly trained faculty in clinical and basic sciences and commitment and capability to provide guidance to clinically trained individuals in the development of independent research careers. The environment desired is one which will stimulate and increase the interaction between basic scientists and clinical investigators. Candidates must be nominated by an institution on the basis of qualifications, interests, accomplishments, motivation, and potential for a research career. Evidence of the commitment of the institution to the candidate's research and development must be provided. Three letters of recommendation attesting to the candidate's academic qualifications, motivation and research potential should be submitted. 2. The Program The individual's program should be designed in two phases. The candidate and the sponsor are jointly responsible for the preparation of the research development plan. It should start with a creative and detailed basic science learning experience in Phase I and progress to an intensive research activity in Phase II under the general guidance of a qualified sponsor. The sponsor may form an advisory committee, similiar to a graduate student training committee, to develop a Phase I program for the candidate which should include course work, seminars, initial research experience, and other educational experience necessary for intensive research in Phase II. The criteria for the transition of the candidate from Phase I to Phase II may also be developed by this committee. Only a general research plan for Phase II is required at this time. Awardees and their sponsors will be required to submit a special, detailed progress report at the end of Phase I. This report is to contain specific information concerning progress and accomplishments and, in particular, an appropriately detailed Phase II research plan and protocol for administrative review and approval. 3. Duration and Effort This five year non-renewable award is based on up to five full-time 12 month appointments. All funds must be used on behalf of the original candidate. It is required that a minimum of 75 percent effort be devoted to the research and research training program. The balance of effort can be devoted to other clinical and teaching pursuits only if they are consonant with the program goals, i.e., the awardee's development into an independent biomedical research investigator or necessary to maintain clinical skills necessary for an academic clinical career. It is desirable for individuals to complete both phases without interruption. It may be permissible, however, to interrupt the award and delay the start of Phase II in order to engage in further clinical training. In the event such a contingency arises, the awardee and the sponsor must justify the interruption to the awarding institute to assure that funds will be available to resume the award so that the candidate may complete the program. 5. Allowable Costs a. Salary -- Individual compensation based on the institution's salary scale for residents or junior faculty at an equivalent experience level but funding from this award for salary not to exceed $40,000 per year per individual plus commensurate fringe benefits for essentially full-time (75-100 percent) effort to the endeavor.* b. Sponsor's Support -- A sum of up to 10 percent of the primary sponsor's salary and commensurate fringe benefits during Phase I. c. Research and Development Support -- $10,000 per year in Phase I increasing to $20,000 per year in Phase II for research project requirements and related support, e.g., technical personnel costs, supplies, equipment, candidate travel, medical insurance premiums and tuition for necessary courses. d. Indirect Costs -- reimbursement of actual indirect costs at a rate up to, but not exceeding, 8 percent of the total direct costs of each award, exclusive of tuition, fees and expenditures for equipment. 6. Concurrent Awards Individuals entering Phase II are encouraged to apply for additional research support, e.g., First Award (R29) or Research Project Award (RO1). Such support may be applied for and held with no reduction in the $20,000 provided as research support. However, salary support from PHS sources above the $40,000 provided by this award is not allowable. *NIH policy encourages supplementation from non-government sources, e.g., voluntary or professional organizations. B. Program Award Institutions with Program Awards may recruit and select candidates into their programs on a local basis rather than submitting a separate application on behalf of each prospective candidate. In all other respects, Program Awards are intended to provide support for the development of physician scientists in the same manner and under the same terms as the individual awards. 1. The Environment Applications will be accepted from an association of departments and divisions and/or clinical departments representing a range of research interests. The grantee institution must be a domestic university, medical school, or comparable institution with strong, well-established research and training programs with adequate numbers of highly tained faculty in clinical and basic sciences and with the interest and capability to provide guidance to clinically trained individuals in the development of research independence. The environment sought is one which will stimulate and increase the interaction between basic scientists and clinical investigators. 2. Program Director The proposed Program Director should possess the scientific expertise, leadership and adminstrative capabilities required to coordinate and supervise an interdisciplinary research and development program of this scope. The Director should also be experienced in the design and management of programs for developing investigators, and should be able to demonstrate a superior record in the preparation of clinical investigators for independent research. In addition, a committee with representatives from the appropriate basic and clinical science departments shall be established to advise the Program Director. 3. Sponsor Each candidate appointed on the grant must have a primary sponsor who is recognized as an accomplished investigator, actively involved in basic science research who will provide the guidance for the candidate's development and research plan. The primary sponsor must be committed to continue this involvement through the individual's total period of development under the award. In some cases candidates may elect to have a secondary clinical sponsor for the research intensive years. 4. Program The Program award provides five years of renewable support. The award is intended to provide up to five years support of consecutive full-time 12 month appointments to each individual candidate appointed. This support is divided into two distinct phases that relate to the individual's progress in becoming an independent investigator. The support starts with Phase I which is to be a creative and detailed basic science learning experience and culminates in Phase II which requires intensive research under the general guidance of a qualified sponsor. It is desirable for individuals to complete both phases without interruption. It is permissible, however, to delay the start of Phase II in order to engage in clinical training. In the event such a delay occurs, it is expected that the program director will plan to provide the necessary resources for the awardee to reenter and complete the program. Awardees and their sponsors will be required to submit a special, detailed progress report at the end of Phase I. This report is to contain specific information concerning progress and accomplishments and, in particular an appropriately detailed Phase II research plan and protocol for administrative review and approval. 5. Budgeting for Future Years Critical to the success of this program award is the ability of the Program Director to make detailed mid-course assessments of each candidate's developing research skill and of the proper time for transition from one phase to another. It is expected that applicant institutions will initiate their activities under this award in a staged manner. That is, the first requested year of support would include funds for candidates in Phase I only. The second year would request funds for new candidates in Phase I as well as for continued funding of the first year's supported individuals. In this way, the requested level of support would increase steadily from the 01 through the 05 budget period as new candidates were appointed. 7. EVALUATION Awardees must agree to inform the National Institutes of Health annually for a period of five years subsequent to completion of the award about academic status, publications, and research grants or contracts received. IV. MECHANISM OF SUPPORT The mechanism of support for this activity will be the research grant, awarded under the authority of the Public Health Service Act, Title III, Section 301 (Public Law 78-410, as amended, 42 USC 241). The regulations (Code of Federal Regulations, Title 42 Part 52, and Title 45 Part 74) and policies which govern the research grant programs of the National Institutes of Health (NIH), will prevail. The award of grants pursuant to this announcement is contingent upon availability of appropriated funds. V. METHODS AND CRITERIA OF REVIEW Applications will be received by the NIH Division of Research Grants (DRG), and, governed by normal programmatic considerations as specified in the NIH Referral Guidelines, will be assigned to the appropriate institute for possible funding. Applications in response to the Announcement will be reviewed in nationwide competition, and in accordance with the usual NIH peer review procedure. They will first be reviewed for potential for research development and scientific and technical merit by an institute review group composed mostly of non-Federal scientific consultants (initial review group). Following this review, the applications will be evaluated by the appropriate Institute Advisory Council (IAC). A. The criteria for initial review of applications include: 1. Candidate - Candidate's competence in clinical activities, potential for a career in independent research, and commitment to, or interest to pursue a research career. 2. Sponsor - The sponsor's accomplishments in the basic science research area(s) proposed, and, where applicable, the accomplishments of the secondary clinical sponsor (Phase II); experience of the sponsor in training students; commitment of the primary sponsor for the duration of the candidate's development and research plan. 3. Environment - The institution's ability to provide adequate facilities, resources, and opportunities necessary for the candidate's training; the quality of the faculty in the clinical and basic sciences, and the extent of interactions between these faculty members; and the quality of the institution's research and research training programs. 4. Training Plan - The adequacy of: a) the plan for the Phase I (didactic) program; b) criteria for determining when Phase I has been successfully completed; and c) the overall plan for research experience in Phase II. Biographical Sketch INDIVIDUAL AWARD: Individuals must be U.S. citzens or noncitizen nationals of the United States, or have been lawfully admitted to the United States for permanent residence and have in their possession an Alien Registration Receipt Card (I-151 or I-551). Candidate should also include information about education, any past military service, internships, residencies, research assistantships, fellowship awards, prizes, publications and activities pertinent to the proposed program. PROGRAM AWARD: Individuals appointed under the Program Award must meet the same requirements cited for Individual Awards. Section II INDIVIDUAL AWARD Research Plan: The laboratory research experience in Phase I should be oriented around a specific research project(s), either ongoing or to be initiated. Because of the strong element of research training sought in Phase I, details should be provided on the specific didactic work to be required of the candidate. Other plans for training and development, e.g., attendence at seminars, special group meetings, should be described. It should be stated whether or not the program is designed to allow the candidate to earn the Ph.D. Candidate and sponsor together should describe the research plan using instructions as outlined on pages 19 and 20 of Form PHS 398. The Phase II research plan should be as detailed as possible at this time. Where it is premature to provide a detailed research plan, the candidate should discuss the area in which the research will be performed and provide whatever information supports the feasibility, relevance and significance of the anticipated research. In addition to the narrative for the Research Plan, candidates and sponsors need to prepare the following: Candidate: A section should be included on (1) those aspects of the candidate's background and experience that support participation in the Physician Scientist Award; (2) a summary of immediate and long-term research career objectives, noting how the award would contribute to their attainment; (3) an estimate of the percentage of the candidate's total work effort to be devoted to research, academic studies, clinical practice, consultation, and other activities, such as, formal graduate studies and seminars. At least three letters of recommendation specifically addressing the candidate's potential for a research career should accompany the application. Sponsor: This section should describe the sponsor's (1) specific plans for supervising, guiding, and counseling the candidate and (2) the criteria by which the candidate will be deemed ready to move to Phase II of the award. The sponsor should also summarize the intellectual environment to illustrate the strength of the clinical and basic science faculty, the interaction between basic and clinical scientists, and the opportunities for enhancing research career development. In addition to his/her scientific expertise, the sponsor should indicate his/her past experience in supervising the training of individuals for basic biomedical research by providing examples of scientists trained and their current professional status. PROGRAM AWARD Research Plans: A research and development plan should be presented using the instructions for Form PHS 398 and the supplemental information required above for the research plan of an Individual Award. A discussion of how research and development plans will be formulated to meet the individual needs of each candidate selected should appear in this section along with a description of sample plans including descriptions of the didactic experiences, e.g., core courses, seminar attendance. This section should include (1) specific plans for supervising, guiding, and counseling the candidate; and (2) the criteria by which the candidate will be deemed ready to move to Phase II of the award. Candidates: The characteristics sought in selecting candidates that support their participation in the Program Award should be described together with an estimate of the percentage of the candidate's total work effort to be devoted to research, academic studies, clinical practice, consultation, and other activities, such as, graduate studies and seminars.
