No, many points in the outline are missing. Just look at any Major section. e.g. Chapter VIII. Section VII. Only (g), (h), (i)

I've only gotten 1 out of 5 to show up. The outline in the early pages is what needs to be counted - every one of those outline points has a reference. Have you added those Bookmarks to the pdf?

I think small web pages would be best - the quickest. Not all can handle a 30meg download. Think of those in the third world. Plus the fact that html pages can be used inside other applications. A pdf is useful on one's own system. The problem I've had with the pdf is that I cannot get any pdf print...

The Word Doc is the source at this point. All exports are full of crud,even when they are cleaned up by MS Word HTML cleaners.
