I think you are correct in understanding this
preposition as "inside." EN to me conveys "in a
sphere." EIS would seem to occupy a space "deeper."
And since it is used of the body and in opposition to
EKTOS, I find your "spiritual cancer" metaphor most
intriguing, if not altogether unique.
I'd run with that for now.
