[You can omit the name, since it is in the byline. You can use your agent's name here if you have one.]

That's it. A clean, simple title page. No copyright notice and no WGA number are necessary, though you will have registered your script at least before you send it anywhere.

Notice how simplicity and a clean conservative look are the industry standards.

When I was playwright-in-residence at the New Rose Theatre, we once got a script in the mail that included a photograph of the playwright - in her bikini.

No. Don't. It doesn't work. And neither do any of the other "attention getting" schemes.

Let your story make the impression, not the package.
