hH EIS EME functions adjectivally in relation to hH XARIS AUTOU.  As such it is an attributive adjective in the second attributive position.  It would be possible to translate it as "His toward-me-grace" or "His grace which is toward me."  It is not really an appositive as you asked since it does not say the same thing or refer to the same thing in different words.
