Your database headers look good, and I would like to offer a few more for
consideration.

The age of the tree might be more easily used from year to year if the
planting date were recorded, rather than the age of the tree in years.
Other info --
The type of planting space: Treelawn (tree + turf); Pit ("cutout" of
sidewalk and curb); Planting bed (tree + shrubs +/or groundcover);
Container (raised in some fashion)
Planting space size: length x width
Vacant space: an existing planting space with no tree, + estimate of size
of tree that might grow well there (small/medium/large)
Distance from trunk to nearest edge of planting space
Distance from trunk to nearest building
Adjacent building height
Any conflicts with wires, signs, buildings, machinery

This is the type of information an urban forester needs in order to
schedule work.  If your interest is only in recording which types of trees
grow where, then these additional items may be unnecessary.  The size and
type of planting space is useful information, however.
